Last day to register campers and complete submission of required documents (health and immunization forms) and fees will be no later than the Tuesday before your child is scheduled to attend. There will be no exceptions made to this policy.


STEP 1 - Complete the online registration form 





STEP 2 - Print, read and sign the



STEP 3 - Gather the following documents and submit these to the ILS Camps Office.

Forms may be submitted in person or via email to


  1. Legible copy of each camper’s birth certificate or passport.

  2. Original Florida Department of Health: Florida Certification of Immunization (Form DH680)

  3. Original Florida Department of Health: State of Florida School Entry Health Exam (Form DH340)

  4. Camper picture (face shot)

  5. Any report card from the 2019-2020 school year


STEP 4 -  Prepare form of payment in cash, credit card, cashier's check or money order made out to Immaculata-La Salle High School. NO personal checks accepted. Payments may be made by phone with credit card. 

STEP 5 - Visit ILS Camp Office with payment and ALL required forms if you haven't done this electronically. 



If the need for cancellation arises, the following policies apply:


  * Cancellation prior to the camper’s first day of scheduled arrival will result in a cancellation charge equal in value to 50% of all cancelled weeks’ fees.


  * Cancellation after a camper’s first day of camp will result in no refunds for any registered camper.


Include lunch, snack, camp t-shirt, field trips and all camp activities with the exception of optional allowances for off campus activities.
